Donna Jean Hoffman, age, 69 of Liberty, KY passed away on Monday October 18 ^th , 2021, at the Ephraim McDowell Regional Medical Center. She was born March 21 ^st , 1952, in Warwick RI, a daughter of the late Anthony V. and Edna Mae Thomas Mattos.

Survivors include: Her Spouse: Carl James Hoffman, One Son: Carl Hoffman Jr of Liberty, Eleven Grandchildren and Seven Great Grandchildren.

Preceding her in death besides her parents was One Daughter: Jennifer Lynn Hoffman and One Brother.

Cremation Rites were honored by Bartle Funeral Home.
